Discussing Your In-Home Care Needs and Personal Care Plan

We welcome the opportunity to conduct a FREE in-home Assessment to meet with you, your loved one, observe their residence, and discuss particulars in developing an effective "in-home Care Plan" - a strategy for helping your loved ones ensure continuity of care and services, continued good health, improved quality of life, reduce the need for hospitalization or institutionalization, and enable them to live independently for as long as possible.

With direct input from you, your loved one, and our companion caregivers, a collaborative custom Care Plan will be developed to ensure shared vision of needs and expectations for the type of care and services are to be rendered and in what frequency. Care Plans are regularly updated and available for review to confirm regular progress.

What to Expect During an Assessment

Important items our staff would like to discuss with you and your loved one and observe throughout the residence include the following:

Current eating, drinking habits and dietary concerns
Possible impairments with eyesight, hearing, and speaking
Various personal hygiene requirements and expectations
Daily sleeping and resting patterns and habits
Medication reminders and possible wound care
Mobility concerns and possible exercise/physical activity routines
Potential travel arrangements, locations, and frequencies
Household concerns regarding cleaning, laundry, and maintenance
Observe and resolve possible safety and security issues and concerns
Identify key points of contact - family, friends, doctors, or social workers
Review emergency plan in the event possible unforeseen events or accidents

Things to Remember

Our companion caregivers are guest in your home. Maintaining respect for your privacy, residence, and possessions is very important.

For this reason, our staff will not provide services or engage in action that is not collaboratively discussed with you.
